MEDICAL RECORDS

All medical record requests must be made in writing.  Most requests are managed within 10 business days.

If the student most recently attended CSDNB Grades 9 – 12 , Contact Virginia Brown - Guidance Department |(860) 225-6300 |(860) 224-2077 (fax) | brownv@csdnb.org

* Please note, if the student is transitioning to another school district and is between 8th and 9th grades, the records may be found at either NBHS.

The following criteria must be met in order for obtaining records:

Parent Request

* Student must be currently under the age of 18

* Legal documentation showing right to access the records

- Birth certificate

- Transfer of Custody

- Appointment of guardian
