All new registrants for CSDNB need a current physical exam and proof of the state mandated immunizations (up-to-date immunization record). Physicals and immunizations are required for prekindergarten (both 3 and 4), kindergarten, 6th and 9th grade. Please note, the State of Connecticut requires all children entering these grades have an up-to-date physical exam and the required up-to-date immunizations. NO exceptions.

Per Connecticut State mandate, all students must be up to date with their immunizations to attend public schools. In accordance with state law and recently updated Board of Education Policy, students who are not in compliance at the start of the 2026–2027 school year will NOT be allowed to attend school or enter the building. If compliance is not addressed, your child may be subject to disenrollment from CSDNB.
Medical documentation can be sent by email, fax, mail, or you can bring the documentation to your student’s school. If your child is planning on participating in sports, all students trying out for or participating in any sports programs at the High School or Middle School must have an up to date SPORTS physical form on file in the nurse’s office. Sports physicals can be recorded on the State of Connecticut health form. The sports physical form is required every year and is valid for 13 months from the date documented by the medical provider. It is important to get the current physical form to the nurse’s office before tryouts or practices in order for the athlete to be cleared to play. Physical forms are available in the nurses’ office, on-line, or from your child's medical provider.
A CSDNB School-Based Health Center (SBHC) is available to assist families over the summer. Gaffney Elementary School-SBHC, will be open August 10-14 and August 17-21. Families can schedule an appointment by calling 475-231-6978. Students MUST register for services provided by SBHC.
